Co-Chairs
Esha Sawant and Rebecca Lumia are the Co-Chairs for SB HOME. They have been involved in SB HOME leadership since their first year of medical school and have loved every moment of it. Being able to serve the patients of SB HOME has been the most rewarding part of medical school and we are so happy to be leading a wonderful team this year to continue to improve our clinic!
Patient Contact and Scheduling Chair
The Phone Duty team serves as the primary point of connection between patients and SB HOME, ensuring consistent communication and continuity of care. Our team is responsible for addressing patient concerns communicated through phone calls and email, including scheduling appointments, responding to questions regarding lab results and prescriptions, and facilitating the sharing of medical records. Above all, the Phone Duty team is dedicated to supporting our patients by quickly and effectively addressing their needs, questions, and concerns.

Pharmacy and Patient Follow-Up Chair
Our Pharmacy and Patient Follow-Up Team is dedicated to ensuring that patients receive their medications promptly and at an affordable cost. We help identify cost-saving options, including GoodRx coupons, and work with pharmacies to find affordable medication options available. Following each clinic, our team contacts both the pharmacy and the patient if needed to identify and resolve any prescription-related issues, helping ensure that every patient has access to the medications they need.
Our Patient Follow-Up Team works with patients who were unable to attend their appointments to identify any barriers to care and assist them in rescheduling. The team also reaches out to patients who are due for follow-up visits, and helps them schedule to uphold continuity of care.

Continuity Chair
The Continuity team helps patients access the follow-up care and services they need after visiting Stony Brook’s student-run free clinic. For patients who are uninsured or facing financial barriers, the team works to connect them with free or low-cost appointments across a wide network of referral sites, including Stony Brook specialty departments. These referrals may include specialty medical care, bloodwork, imaging, preventive health services, mental health care, and other supports based on each patient’s needs. Continuity also works with Social Work interns to help patients navigate available financial assistance resources at Stony Brook to lower the cost of getting the care they need.

Volunteer Coordinator Chair
The Volunteer Coordinator serves as the central point of contact for all volunteer-related operations at SB HOME, overseeing the schedule for every role in clinic: preclinical students, clinical students, clinic managers, and beyond. This role is responsible for scheduling volunteers for each clinic date, training clinic managers, ensuring all volunteers stay current on required clinic trainings, and coordinating swift, seamless coverage in the event of last-minute cancellations. The Volunteer Coordinator also serves as the point of contact for all volunteer-related inquiries and communications, working closely with clinic managers and sub-committee heads to keep clinic operations running smoothly. Above all, the Volunteer Coordinator is dedicated to building a reliable, well-trained volunteer base so that patients receive consistent, high-quality care at every clinic.

Specialty Clinic Chair
As a specialty clinic chair of SB HOME, I help organize additional collaborative clinics with other departments at Stony Brook University Hospital throughout the year. In previous years, we have collaborated with AccessGYN, which provides gynecological care for our patients, ranging from Pap smears to breast exams. Because many of our patients have diabetes, we also collaborate with the Stony Brook Endocrinology team to provide specialized care for our high-risk diabetic patients. I help coordinate care between the Endocrinology team and the providers and clinical students at SB HOME to ensure that our patients receive the best individualized care possible. Additionally, we have collaborated with the Stony Brook Ophthalmology team to provide specialized eye care, such as retinal exams and vision assessments, particularly for patients living with diabetes.

Finance Chair
As Finance Chair for SB HOME, I oversee budgeting and help identify opportunities to reduce clinic expenses and improve the use of financial resources. I also work to optimize patient laboratory billing costs and coordinate major fundraising initiatives, including the annual gala and volleyball tournament, to support the clinic’s operations and patient care. I oversee food pantry operations at the clinic, collaborate with the Nutrition Committee to strengthen food access initiatives, and work with the HOPE Program to organize educational and clinical experiences for local high school students interested in healthcare. I also organize community outreach events, including health fairs, screenings, and mental health education programs, while coordinating outreach-related volunteers and engaging students. Through these efforts, I work to strengthen SB HOME’s connections with the communities we serve and expand access to health resources, education, and meaningful opportunities.

Operations
The Operations Team helps keep SB HOME organized and running smoothly behind the scenes. We maintain the SB HOME website to ensure that patients have access to accurate information, announcements, and resources, and we use feedback from patients, volunteers, and preceptors to identify opportunities to improve the clinic. The Operations Team also supports clinic communication and community engagement through our social media platforms and works closely with other SB HOME teams on projects that improve the patient experience. Together with the Volunteer Coordinator and Outreach Chair, we also help support clinic managers and student representatives to ensure that clinic operations run efficiently.
