Directions
Take Long Island Expressway to Exit 62 (Nicholls Road. Head north on Nichols Road. Continue straight on Nicholls Road, heading towards Stony Brook. Make a right onto Route 347 East. At the third traffic light, make a left onto Belle Mead Road. After the first traffic light you will see an entrance to the Primary Care Center on the right. Turn into Stony Brook Primary Care Center Parking lot. Our office is on the far side of the building.

Online Patient Requests via Patient Portal: www.stonybrookmedicine.edu
We offer an online Patient Portal through our electronic medical record. The patient portal is an electronic gateway which allows you to send and receive messages from your doctor. The Patient Portal is intended to be used for NON-URGENT messages. You can request medication refills, you can request an appointment, or you can send a general message to your doctor or doctor’s staff. Not only can you send messages on the portal but you can view and print test results and educational information given to you by your doctor.
Please note that the portal is not intended for urgent messages; urgent messages require you to call the office. There can be a delay of one or two business days for portal messages so the portal is NOT intended for communicating new symptoms, acute medical problems or the need for immediate medication renewals.
To establish your Patient Portal account, speak with our office staff during your next visit to the office.
