The Department of Medicine is pleased to announce that once again Stony Brook Primary Care in East Setauket and Stony Brook Internists, Commack office, have achieved NCQA accreditation for PCMH evaluation in 2024. This year, the Stony Brook Internists, Lake Grove office, joins the ranks and has officially received notification for accreditation by NCQA as well.

NCQA’s Patient-Centered Medical Home (PCMH) Recognition program is the most widely adopted PCMH evaluation program in the country. More than 10,000 practices (with 50,000+ clinicians) are recognized by NCQA.
